NYC Health + Hospitals | Gotham Health MorrisaniaDiagnostic and Treatment Center serves the South Bronx through a two-pronged approach: providing treatment to those who need it, while at the same time emphasizing prevention. Morrisania runs one of the oldest School Health Programs in New York City and serves four schools through five school-based clinics. These clinics ensure access to free primary medical, dental and mental health care to children who have unmet health needs.

 

Job Description

 

Works regionally in conjunction with Gotham Health administrative and medical leadership to develop and implement the strategic plan of the assigned regional community health sites. Acts as a change agent, leader and role model for the nursing profession with the goal of having nurses work at the top of their licenses. Provides guidance and develops action plans to address patient care issues. The aim is to address and remove variation and fragmentation through education that may exist and strive to standardize through the adoption of best practice ensuring that all services are of the highest quality and implemented Gotham wide.

  • Assess and architect the new culture by articulating the desired mindsets and behaviors that must be in place for Gotham’s Health Nursing transformation to be fully realized.
  • Develop and execute an evidence based education plan that will drive change and allow stakeholders to understand, embrace, and institutionalize the desired new mindsets and behaviors.
  • Provide highly visible individual leadership to support and promote a framework conducive to stability and consistency providing a baseline upon which a process sits within Gotham Health.
  • Building a Gotham Nursing Commitment that foster all levels of nursing and it being committed to achieving Gotham Health’s transformation. This includes articulating levels of commitment needed, assessing commitment, and planning the actions necessary to drive higher levels of commitment from key individuals, clinics and constituents.
  • Uses evidence-based patient safety methodology to support, develop, educate and implement initiatives across the health system.
  • Ensuring the standardization in developing, implementing, and enforcement of all policies and procedures, including patient rights in an outpatient primary care clinical setting.
  • Educates technical and patient care staff in the use of new equipment, supplies, and instruments; coordinates in-service training and workshops for appropriate staff
  • Assessing adaptation capacity by periodically assessing Gotham Health Nursing adaptation capacity and change load. This includes the ability to understand constituencies that will be affected by specific changes; to understand the nature, degree, and timing of that impact; and to share that perspective with Nursing and executive leadership in a timely manner so that capacity risk can be mitigated.
  • Take responsibility for ensuring that the standard work is adopted and cascaded throughout all areas within Nursing and Gotham Health.
  • Capability to understand and analyze complex issues, drawing on the breadth of data that needs to inform system wide deliberations and decision-making, and the wisdom to ensure that it is used ethically to balance competing priorities and make difficult decisions.
  • Ensuring the development, implementation, and enforcement of all policies and procedures, including patient rights in an outpatient primary care clinical setting.
  • Planning for, and administration of, the managerial, operational, and reporting components of a primary care.
  • Participating in the quality assurance program for participant care and staff performance. Develops and directs continuous quality improvement programs which achieve targeted patient quality of care outcomes.
  • Take responsibility to comply with applicable laws and regulations.
  • Ensuring the provision of staff orientation, staff education, and ongoing staff training in accordance with state regulations.
  • Ensuring all health care workers are protected from biological hazards consistent with state federal and CDC Guidelines.
  • Ensuring an OSHA/Infection Control program, records of work injuries, and employee health records are maintained and managed appropriately.
  • Ensures all clinical personnel practice infection control and standard precautions maintain compliance with all safety regulations.
  • Actively participates in planning and designing policies and procedures in collaboration with appropriate staff members.
  • Identifies work related improvement opportunities and analyzes them to identify causes/find solutions that improve teamwork and patient satisfaction.
  • Acts decisively to implement solutions to resolve critical issues or improve processes.
  • Communicates clear understanding of job responsibilities, task objectives, deadlines and performance expectations.
  • Shares up-to-date information on facility activities and policy changes in a timely manner.
  • Listens attentively to others. Tries to understand the meaning of the communication of others, perceives their needs and interests.
  • Works in harmony with others as a team leader.
  • Participates in on site regulatory surveys, i.e. VFC, Laboratory, Article 28, HRSA and development of Corrective Action Plans as needed.

  • Understands, verbalizes, and participates in the quality improvement process.
  • Monitors, supervises, and submits all mandatory state reporting to the DOH by mandatory deadlines.
  • Supervises, contributes, and participates in all mandatory and time sensitive sites specific RCA.
  • Supports initiatives designed to improve individual and organizational performance.
  • Overseeing the maintenance of all clinical equipment.
  • Overseeing Infection Prevention and Control Program.
  • Regional lead for creative and innovative paths forward in the front lines of nursing and patient care.
  • Must possess an understanding of Gotham’s overall strategic goals and have demonstrate a sense of commitment and passion to achieving those Goals.
  • Responsible for leading and participating in improvement efforts to ensure excellence and greater quality of care across all Gotham settings
  • Data Analytics- a focus on outcomes and improvement
  • Lead initiatives designed to achieve the triple aim of an improved patient experience of care (including quality and satisfaction, improved outcomes and/or health of populations, and a reduction in the per capita cost of healthcare.
  • Responsible for tracking outcomes as a measure of effectiveness

Minimum Qualifications

 

CERTIFICATION(S)/LICENSE(S):

  • Licensed and currently registered to practice as a Registered Professional Nurse in New York State
  • Current certification in Ambulatory Care Nursing, preferred.

Years of Experience: Preferred Seven years of progrressive leadership experience in Primary Care Setting

Education Level: Bachelor of Science Degree in Nursing, Master’s Degree of Nursing or related field required.
