What is a Case Manager? The Case Manager position is the heart of our work at PATH. The Case Manager will work in an interim housing setting (low-barrier, high tolerance) to provide case management services for guests experiencing homelessness and have a variety of complex health, functional, and social needs. These individuals may be exiting from hospitals, emergency rooms, and other health care settings. Interim housing guests are referred to as “guests” and provided with a home-like environment coupled with support and case management services to develop a plan to transition back into the community. The Case Manager will collaborate with other departments internal and external to PATH as new guests are admitted into Interim Housing. The Case Manager will assist these guests in breaking the cycle of homelessness by establishing and pursuing a permanent housing plan and accessing and maintaining necessary support services in the community. The Case Manager will assist guests with building skills, reducing mental health and functional barriers to obtaining and maintain housing, and strengthening community social service linkages necessary for the guest to fully integrate into the community once they leave interim housing.
PATH is a $50M+ non-profit provider of shelter and supportive services for the homeless with locations throughout Southern California, Santa Barbara, and San Jose, primarily serving veterans, families, and the chronically homeless. Funding comes from government agencies, foundations, and individuals, and we currently have over 100 grants from over 30 funders. PATH Ventures (PV) is a non-profit developer of permanent supportive housing with projects located throughout Southern California. PV possesses an ownership interest via limited partnerships in over a dozen residential communities plus several projects in active predevelopment.

  • Process and accept referrals from funders
  • Meet with assigned guests a minimum of 1x/week to support with progression towards goals identified by guest in their ISP
  • Conduct intake and individualized needs assessment for all guests and assist team by collaborating with guests to develop Individualized Service Plans (ISP) that address barriers to obtaining services, housing, and appropriate health care needs
  • Facilitate psychosocial rehabilitation groups for guests as required
  • Conduct a full VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) at admission for all new guests to aid in the Individual Service Plan (ISP) development
  • Conduct and complete all new guest intakes and enter information into HMIS in a timely fashion
  • Provide ongoing case management support with benefits establishment (GR, SSI, TANF, Veterans pension, etc.), obtaining ID, birth certificate, social security cards, provide employment linkages and assistance, housing referrals and health related assistance as identified on the ISP
  • Provide referrals and linkages to primary and behavioral health services and other supportive services
  • Collaborate with referring organizations to coordinate guests’ care and ensure guests receive the services and support they need to achieve and maintain health, mental health, and housing stability
  • Assist with housing applications, complete supportive and subsidized housing paperwork, survey rental market for affordable housing
  • Assist and educate guests as needed about care and safety regarding their animals
  • Advocate with prospective landlords, and identify appropriate permanent housing options for guests
  • Provide ongoing information, referrals, linkages, and advocacy for all other identified needs.
  • Provide linkage, referrals, and case management support to interim guests
  • Use and develop community resources to broker and link guests to services
  • Provide all guests with a home-like environment for the duration of their stay in PATH Interim Housing
  • Collaborate with the case management and operations teams to ensure that the facility is clean and safe
  • Ensure that guests meals are served at the established times and support kitchen staff as required
  • Assist all volunteers by providing all necessary supports to ensure that they have a valuable experience
  • Promote and follow interim housing established values, guidelines and rules
  • Participate in staff meetings and assigned trainings

  • Maintain documentation standards as set forth by the program contract and PATH policies, including maintaining compliance with HIPAA policies and practices
  • Complete progress notes on every face to face/ telephone contact with guest or collateral contact by the next business day at 5pm
  • Complete intake in coordination with guest and within the documentation standards set forth by the program contract and PATH
What You Bring
  • Maintain and execute confidential information according to HIPAA standards
  • Demonstrated knowledge and or experience with case management techniques, including critical time intervention, harm reduction strategies, crisis intervention techniques and motivational interviewing
  • Flexible work schedule to include some evenings and/or weekends as needed
  • HMIS experience preferred
  • Possesses a high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs
  • Ability to work independently and within a team environment and exercise mature judgment
  • Ability to provide community and in-home based services on a regular basis
  • Strong written and verbal communication skills
  • Highly motivated self- starter with the ability to coordinate multiple projects/ tasks simultaneously in a high-pressure environment

Work Environment You may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves. The employee may experience a number of unpleasant sensory demands associated with the guest’s use of alcohol and drugs, and the lack of personal care. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. You will also be in contact with a variety of animals as guests are permitted to reside with their pets during their stay.
