Project Overview: Collaborative Home Modication Initiative for Aging in Place

Charity Care Group, in partnership with the U.S. Department of Housing and Urban Development's Older Adults Home Modication Program (OAHMP), is launching a comprehensive initiative to enable low-income elderly homeowners in Norfolk, Virginia to age in place safely and comfortably.

This collaborative project aims to:

1. Implement low-cost, high-impact home modications to reduce fall risks, improve safety, and increase accessibility for seniors aged 62 and older. Modications may include installing grab bars, railings, lever-handled doorknobs and faucets, temporary ramps, and adaptive equipment.
2. Utilize a person-centered approach, incorporating the expertise of licensed Occupational Therapists (OTs) to tailor modications to each client's specic goals and needs. OTs will evaluate clients' functional abilities and home environments to optimize independence.
3. Serve approximately 50 low-income elderly homeowners over a 3-year period, focusing on those unable to perform modications themselves.
4. Coordinate with local community organizations, healthcare providers, and senior services to identify and reach eligible participants.
5. Provide education and resources on fall prevention, home safety, and aging in place strategies to program participants and the broader community.
6. Leverage Charity Care Group's existing relationships and utilize Certied Aging-in-Place Specialists (CAPS) to maximize program impact and efficiency.
7. Adhere to HUD's guidelines, including the $5,000 per unit cap for modications, while ensuring comprehensive support for each participant.
8. Contribute to the growing body of research demonstrating the positive impact of home modications on fall prevention and disability reduction among community-dwelling elderly.

Scope of Work for Home Modications under the Older Adults Home Modication Program (OAHMP)

Detailed List of Potential Modications:

1. Licenses and Certications Required:
   1. Valid Virginia Class A, B, or C Contractor's License, depending on the scope and value of work
   2. Business License for the City of Norfolk, Virginia
   3. Certied Aging-in-Place Specialist (CAPS) certication from the National Association of Home Builders (NAHB) preferred
   4. Universal Design Certied Professional (UDCP) certication preferred
   5. Lead-Safe Certied by the EPA if working in homes built before 1978

Insurance Requirements:

Budget Guidelines for OAHMP Home Modication RFP

1. Maximum Budget per Home:
   1. As per HUD guidelines, the maximum budget for home modications is

$5,000 per unit.

• 1. This $5,000 cap includes all costs related to physical modications, including labor, materials, and equipment.
  2. Contractors must strive to maximize the impact of modications within this budget constraint.
  3. In exceptional cases where modications may exceed the $5,000 cap, contractors must provide detailed justication for review and potential pre- approval by HUD.

Itemized Cost Breakdown Request:

Contractors must provide a detailed, itemized cost breakdown for each proposed modication. This breakdown should include:

• 1. Labor Costs:
     1. Hourly rates for different types of labor (e.g., carpenter, electrician, plumber)
     2. Estimated hours for each type of labor per modication
     3. Total labor cost per modication
  2. Materials:
     1. Itemized list of all materials needed for each modication
     2. Unit cost for each material
     3. Quantity of each material required
     4. Total material cost per modication
  3. Equipment:
     1. List of any specialized equipment required for modications
     2. Rental or usage costs for equipment

Cost Reporting:
