About the Hospital and the Director Case Management / Care Coordination (RN) position:

• Non-profit, award winning acute care hospital that has served its community for over 50 years and a member of a regional healthcare system.
• The Director will manage a team of 15 FTEs including a Nurse Manager and Social Work Supervisor and will be responsible for the hospital wide Care Coordination Team including Case Management, Utilization Review, Denial Review, and Social Work Services.
• Reports to a visionary Chief Financial Officer who has been with the organization for over seven years.

Director Case Management / Care Coordination (RN) Requirements:

• Bachelor's degree in Nursing (BSN) required / Master's preferred.
• Active RN State of Florida.
• Minimum five (5) years of progressive Care Coordination leadership experience in an acute care hospital required.
• Seeking a RELATIONSHIP DRIVEN, VISIBLE and INSPIRATIONAL leader.
• Proven track record of success in Care Coordination leadership.
• Able to establish positive collaborative relationships across all levels.
