Monday-Friday, 8am-5pm
Call Every 3rd-4th Weekend

Role:
- Lead an interdisciplinary team
- Provide accurate and evidenced based care related to assessing, treating, and diagnosing patients with advanced progressive illness while promoting a healing environment
- Develop and implement appropriate advanced disease treatment plans that effectively manage the patients needs creating a culture with optimal utilization of resources
- Participates in the assessment of medical staff learning needs as they relate to palliative care
- Plans, implements, evaluates educational programs for medical and nursing staff based on identified learning objectives
- Coordinates educational activities with other institutional and community programs
- Responds to changes in clinical practice by planning, designing, implementing, and evaluating guidelines, protocols, and standards in collaboration with other staff
- Participates in the process of program and system development for the management of hospice care
- Leads initiatives to change the structure, system, or processes of the organization to more effectively support hospice care initiatives
Education:
This position requires a Medical Doctorate degree (M.D.), Doctor of Osteopathy (D.O.) or foreign equivalent from an approved school of medicine.
Experience
- Minimum 5 years clinical practice experience, 2 of which must have been in palliative, oncology, geriatric, acute care, and/or hospice care
- Expertise in end-of-life care, including advanced care planning, pain and symptom management of the seriously ill patient, and psycho/social, emotional care issues
