MDS Assessment Coordinator RN

We are seeking an MDS Assessment Coordinator for our Skilled Nursing Facility in Keota, IA. This full-time role involves conducting resident assessments, coordinating care plans, and ensuring compliance with state regulations. The position includes part-time MDS duties and one day per week as a floor nurse.

Responsibilities:

• Assess residents regularly and communicate condition changes.
• Complete MDS, quarterlies, and care plans.
• Prepare and distribute MDS schedules.
• Maintain accurate records and transmit MDS to the state.
• Participate in resident admissions and daily care observations.

Qualifications:

• Registered Nurse (RN) with over a year of MDS experience.

Pay: $40.00 per hour, negotiable.
