We are seeking a dedicated and experienced Consulting Pharmacist to join our team in the Healthcare industry. This position offers a unique blend of clinical and business responsibilities and requires a professional with a deep understanding of pharmaceutical care. The successful candidate will have a proven track record in admixture and will be responsible for providing expert clinical advice, ensuring medication safety, and improving patient outcomes. This is a fantastic opportunity to make a significant impact on patient care while also driving business performance.

Responsibilities:

1. Provide expert clinical advice on the use of medications in various healthcare settings, including hospitals, clinics, and long-term care facilities.
2. Conduct comprehensive medication reviews to identify, resolve, and prevent medication-related problems, including therapeutic drug monitoring and management of potential interactions.
3. Collaborate with healthcare teams to develop and implement individualized medication treatment plans.
4. Ensure the safe and effective use of medications by providing medication information and recommendations to healthcare professionals and patients.
5. Develop and deliver education and training programs to healthcare professionals, patients, and caregivers on medication therapy management.
6. Participate in the selection of cost-effective, evidence-based medications for formulary inclusion.
7. Lead quality improvement initiatives related to medication use, including the development and implementation of clinical guidelines and protocols.
8. Monitor and report on medication utilization trends and outcomes.
9. Ensure compliance with regulatory standards and best practice guidelines related to medication use.
10. Contribute to business development efforts by identifying opportunities to improve medication-related services and patient outcomes.

Qualifications:

1. Doctor of Pharmacy degree (PharmD) from an accredited institution.
2. Current, unrestricted pharmacist license in good standing.
3. Minimum of 5 years of experience as a practicing pharmacist, preferably in a consulting role.
4. Demonstrated expertise in admixture.
5. Advanced clinical knowledge and skills, with a focus on medication therapy management and patient safety.
6. Strong analytical skills, with the ability to interpret and apply clinical data.
7. Excellent communication and interpersonal skills, with the ability to effectively collaborate with diverse healthcare teams.
8. Proven ability to develop and deliver effective education and training programs.
9. Experience with quality improvement initiatives related to medication use.
10. Knowledge of current regulatory standards and best practice guidelines related to medication use.
11. Demonstrated business acumen, with the ability to identify opportunities to improve medication-related services and patient outcomes.
12. Commitment to continuous professional development and learning.
