Primary Care opportunity in Philadelphia, PA

Deliver compassionate, patient-centered, chronic care management to geriatric population in their homes with HarmonyCares, a leader in home-based primary care. Primary Care Physician

• Provide comprehensive primary care to seniors in their homes with a Medical Assistant partner.
• Manage a smaller patient panel (5-6 per day) for personalized care.
• Spend up to 45 minutes per visit to build strong patient relationships.
• Company-provided car for all home visits
• Work Monday-Friday, no weekends.

Physician Leader

• Clinical oversight of Advance Practice Providers
• Oversee clinical operations, mentor providers, and ensure quality standards.
• Maintain a reduced patient panel while focusing on leadership duties.
• Monday-Friday schedule with no weekends.

Requirements

• Pennsylvania  Medical License (or ability to obtain)
• Experience with geriatric chronic care management
• Board Eligible or Board Certified
• Successful completion of residency program required.
