Title: Primary Care Physician

Location: Chicagoland Region

Role Description:

The Primary Care Physician will provide equitable, value-based healthcare to Medicare patients at neighborhood primary care centers. Focused on quality over volume, physicians will work with large care teams to enhance patient experiences and outcomes.

Schedule: Full-Time, Monday to Friday, 8:00 AM - 5:00 PM

Responsibilities:

• Assess and diagnose patients
• Oversee primary care administration
• Prescribe medications
• Maintain electronic medical records
• Collaborate with leadership to meet health goals

• Required Qualifications:

• M.D. or D.O. Graduate
• Board Certification in Internal or Family Medicine
• Active State License and DEA license

• Preferred Qualifications:

• Fellowship in Geriatrics
• Experience with geriatric populations
