Primary Care Physician - Physician Residents 2024-2025

We are seeking a Primary Care Physician to deliver equitable, value-based healthcare to Medicare patients at our neighborhood primary care centers. Physicians will work within a supportive care team, focusing on quality over volume, and will be rewarded for positive health outcomes. Responsibilities include patient assessment, care administration, and collaboration with leadership to meet health goals.

• Required: M.D. or D.O. degree, board certification in Internal or Family Medicine, active state and DEA licenses.
• Preferred: Fellowship in Geriatrics, experience with geriatric populations, and value-based care.
