Job Summary:

We're looking for aRegistered Nurse with experience in healthcare to join our team full-time. This position is remote from your home office. You will be responsible for providing operational and leadership for our telephonic/virtual Remote Patient Monitoring support and Chronic Condition management department. We currently serve senior patients with chronic conditions.

Responsibilities:

You will be responsible for managing a remote interdisciplinary team of medical assistants, nurses, and assisting your team to meet their goals, deliver exceptional customer service, and work within your scope of practice to provide evidenced-based education, assessments, and care navigation. You will model exceptional leadership, drive operational excellence, manage staff productivity, administrative functions, and work to support our overall daily people and workplace operations. You will be supported by clinical leadership for clinical escalations.  

Ideal Candidate:

  • Has experience leading teams of clinicians or health tech teams (LPN/ MA/) >2 years
  • Is confident managing change and has a growth mindset
  • Is comfortable with digital technology (including tools like MS Office, Google, various EMRs, etc.)
  • Is familiar with Medicare, chronic conditions management (CCM), remote patient monitoring (RPM)
  • Has digital health/chronic care management experience
  • Is compassionate and good at listening to patient or staff concerns
  • Has a high level of process-orientation, organization, and resourcefulness
  • Is organized, efficient, and adaptable: able to carry out a variety of administrative and clinical duties
  • Has the ability to think quickly, solve problems, and bring professionalism to all situations
  • Is able to contribute to quality improvement and process improvement initiatives, demonstrates a growth mindset
  • Is goal-oriented
  • Is professional, fun, polite, and has excellent communication and people skills

Requirements:

·         Qualified candidates must be Registered Nurses with a valid NY RN license in good standing with the state

 Experience:

Demonstrated knowledge of ACO quality measures, have leadership experience managing different types of people.

Job Type: Full-time

Schedule:

  • Monday to Friday

Salary: $85,000.00 - $95,000.00
