Providence is calling a RN Clinical Liason- Home Health Services (1.0 FTE, Days Full-Time 40 hours) to Providence Benedictine Home Health serving the Mid-Willamette Valley,OR area.

This opportunity has a Sign on bonus of $5,000, that will be paid out in 2 installments over the spand of 12 months from your start date.

We are seeking a RN Clinical Liason - Home Health Services who will represent Providence Benedictine HH in the communities we serve. This position requires strong communication and interpersonal skills. The role of the Clinical Liaison is to communicate our services, to coordinate and arrange home care with hospitals and facilities associated with a clinical referral .

In this position you will have the following responsibilities:

  • Coordinate care for all services being provided after the Nurse Case Manager/or Discharge Planner provides the patient with freedom of choice for a home services agency and contacts the Liaison with an order for Home Services.

  • Meet with the patient to explain home services being provided and obtains necessary documentation to provide care. Assesses that services ordered are appropriate.

  • Enter patient data and appropriate documentation into the appropriate Home Health database

  • Coordinate with Home Health intake and scheduling staff to determine start of care date for home services


Required qualification for this position includes:

  • Current, unencumbered Oregon RN license required

  • Current Basic Life Support (BLS) for Provider Certification or ability to obtain in 30 days. Renewal required every 2 years

  • One (1) year full-time experience in home health or discharge planning

  • Experience in home care and in positions that required extensive customer service skills.

Preferred qualification for this position includes:

  • Bachelor's Degree in Nursing

  • One (1) year full-time experience in home infusion
