Description

The Social Worker will provide comprehensive health services to end stage renal disease patients, family, and staff through assessment, evaluation, counseling and referral for psychosocial, emotional, and economic issues related to the physical and mental health, end stage renal disease process, dialysis treatments, and life events at Trinity- St Josephs KDU Unit, Minot, North Dakota and Trinity Lenior KDU Unit, Belcourt, North Dakota. The Social Worker will provide social work services and discharge planning to patients and families to meet individual health and psychosocial needs in the promotion of wellness and cost-effective outcomes. Supports existing case management services by improving access to needed services through assessment, counseling, referral and professional functions.

Qualifications

• Licenses and Certifications Required • Current Licensed Certified Social Worker in the state of North Dakota required. • BLS certification preferred. • Educational Requirements • Masters of Social Work degree from a social work school accredited by the Council of Social Work Education in accordance with the plan of care standard 42 CFR 484. • Experience Requirements • Previous experience in Medical Social Work or healthcare preferred. • Special Skills or Training Requirements • Knowledge of laws related to abuse, neglect, minors, etc. along with public and state programs and other community resources. Fundamental communication and interpersonal skills with all levels of internal and external customers, including but not limited to medical staff, patients and families, clinical personnel, support and technical staff, outside agencies, and members of the community. Basic understanding of reimbursement issues and Diagnostic Related Groups (DRGs), and personal computer skills to organize and relay information in meaningful ways using email and word processing.
