Under the general supervision of the Manager of Care Management the Care Management Supervisor performs all the functions of a Care Manager Registered Nurse functions in a supervisory administrative and/or program management capacity within the Department of Care Management.
Performs all duties of the Care Manager and demonstrates expertise in clinical specialty.
Provides clinical supervision and work direction to Care Management staff.
Coordinates work assignment of employees and ensures adequate coverage for care management area of responsibility.
Supports all through-put and capacity management activities for TMH and the Lifespan System.
Communicates policy and procedures to area personnel.
Provides input on hiring.
Provides input to policy & procedure development.
With Manager of Care Management evaluates employee work performance through review and completes annual performance evaluations as required.
Participates in counseling of supervisees around performance issues as required.
Participates in developing schedules and coverage plans.
Participates in system-wide 24/7 coverage for clinical and/or scheduling issues.
Maintains records prepares reports and composes correspondence relative to supervisory administrative and/or program management functions as necessary.
Provides care management consultation within and outside the hospital.
Participates in departmental studies grants Operational Excellence Projects committees and quality assurance activities.
Maintains and enhances professional expertise through educational activities.
Adheres to Lifespan Hospital and departmental policies and procedures to maintain a safe environment of care and compliance with all laws regulations and standards.
Qualifications - Education:
Bachelor s Degree in Nursing from an accredited National League of Nursing (NLN) program with current license to practice as a Registered Nurse in the State of Rhode Island required.
Certification by a recognized national organization in case management is preferred; or certification to be obtained within eighteen months of hire.
Qualifications - Experience:
Three to five years of clinical/care management experience preferably in the acute and/or community setting with an emphasis on populations to be served and in utilization management.
Demonstrated knowledge and skills necessary to provide patient care and to develop or administer care management services with consideration of stages of human development and cultural patterns.
Analytical skills necessary to evaluate patients programs and procedures.
Administrative and supervisory skills necessary to address care management staff functions in the department and/or assigned clinical area.
Demonstrated ability to exercise independent judgment. Refers specific complex problems to Manager of Care Management or designee where clarification of departmental policies and procedures may be required.
Interpersonal skills necessary to establish and maintain effective working relationships and to supervise and coordinate care management activities in the clinical area.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
Works in a normal office environment with exposure to patient care.
Will supervise care management staff as appropriate for clinical service area.
