Compensation:
Pay Range: %2447,665.00 - %2477,455.00/year.

Job Description:
To provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. To provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare. To facilitate discharge planning process and identifies community resources to assist patients and their families.

Bachelor's degree in Social Work and ten (10) years of related experience required. Work experience may NOT substitute for education requirement. Currently licensed as a Licensed Baccalaureate Social Worker (LBSW) by the Alabama State Board of Social Work Examiners required.
