The Transitional Care Coordinator (TCC) is responsible for proactively coordinating and assisting with transitioning medically complex patients from the hospital to home. The TCC will work with the high risk patients to optimize recommendations to ensure continued health with the goal of preventing readmission. In collaboration with the physician, hospital care teams and family/significant others, the TCC will assess evaluate, and implement a plan of care for the patient. The TCC works collaboratively with the Transitional Care Program Manager, Transitional Care Social Worker, Case Management staff and other members of the Multidisciplinary team to develop a Continuum of Care plan to assure patients have the resources and instructions to carry out their Plan of Care safely. The TCC will follow up with the patient and patient support structure to ensure compliance with the medical treatment plans.

1. Education: A minimum of an Associate Degree in nursing (AND) required. Bachelors of Science in

Nursing (BSN) preferred. Effective 1.1.20, BSN or higher nursing degree required within two and one half (2.5) years for all new hires or transfers.

2. Licensure: Current CaliforniaRegistered Nurse license required.Current BLS/Healthcare Provider status

as per American Heart Association standards required.

3. Experience: Minimum of 3 years nursing experience required. Ability to demonstrate a working

knowledge of community resources, post-acute care coordination, and case management principles

required. Bilingual in Spanish preferred. Case Management experience preferred. Broad general

knowledge of nursing and possess the ability to effectively navigate and utilize a computerized medical

record system.

4. Essential Technical/Motor Skills: Demonstrates sufficient manual dexterity to operate office equipment,

including computer terminals, printers, copiers with reasonable speed and ability to move throughout the

Hospital. Experience in data entry, analysis and report writing.

5. Interpersonal Skills: Possess strong interpersonal and leadership skills and have the ability to work well

in a fast-paced team environment that is patient-centered. Ability to relate effectively, tactfully and

professionally to co-workers, physicians, patient and other staff members. Self-directed; organizational and

time management skills necessary to set priorities and efficiently complete the assigned workload.

Effective verbal and written communication skills, including presentation skills.
