Job description :

POSITION OPENED FOR A PREDETERMINED CANDIDATE!!

MUST have a clear driving record, active driver’s license and current auto insurance. Must provide proof within 48 hours of offer acceptance!

6 patients per day/Productivity of 30 per week
Must complete revisits, SOC/ROCs, recerts and discharges
Mileage reimbursed at $0.62 cents per mile

Territory: Phila County - Specific Zip Codes (19118, 19119, 19150, 19126, 19138, 19120, 19140, 19141, 19144, 19129, 19128)

A Home Care Visiting RN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The RN educates regarding self-management and provides support to meet the needs of the patient, family, and community.

Submission Requirements

1+ years of recent Home Health experience - ]REQUIRED]
Case management experience - [REQUIRED]
Homecare Home Base EMR - [REQUIRED]
Previous travel experience - Highly Preferred
IV and venipuncture experience  - Highly Preferred
