The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals. The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.ESSENTIAL JOB DUTIES/RESPONSIBILITIES:<ul><li>The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.</li><li>The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.</li><li>It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).</li><li>The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.</li><li>The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.</li><li>For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.</li><li>PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.</li><li>Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.</li><li>Performs other duties as assigned and modified at manager’s discretion.</li></ul>KNOWLEDGE, SKILLS & ABILITIES:Competencies for Success<ul><li>Availability and Accessibility for patients to build trust from their patients. It is expected that PCP’s will make themselves as available to their patients as possible by being open and available for walk-in visits and answering phone calls and messages in a timely manner.</li><li>Service Orientation — PCP’s provide care that they would want for a family member or for themselves to each patient at every interaction.</li><li>Evidence Based Medicine — The PCP remains updated on evidence-based medicine, but also recognizes that factors outside of traditional medicine, like lifestyle and nutrition, have a large impact on patient health outcomes. The PCP stays up to date on clinical, nutritional, and lifestyle-based interventions to improve outcomes.</li><li>Physician Leadership is integral to good healthcare, so the PCP must be willing to continuously work to develop and improve leadership skills for the benefit of one’s patients, their team, their center and the company.</li><li>Quality — Our patients deserve the highest quality of care. This requires a willingness to work with the care teams towards achieving high quality outcomes and quality measures. At the same time, PCPs will always be looking for ways to continuously and systematically improve their practice of medicine and the operations of their center.</li><li>Influence — PCP’s must competently and compassionately influence their patients, their teams, and themselves to achieve the best outcomes.</li><li>Self-Care — A PCP can take the best care for their patients when they are adequately caring for themselves. That means physically, mentally, socially and spiritually. The role provides a significant opportunity for growth within the firm over time with multiple career paths.Position Responsibilities:Client Reporting & Business Development Support<ul><li>Assist the team in updating quarterly fixed income market outlook and commentary.</li><li>Lead the completion of quarterly and annual information requests from existing clients and partners.</li><li>Regularly distribute Bartlett’s fixed income strategy information and performance details to key consultants and institutional partners.</li><li>Quarterback the completion of institutional RFPs and related information requests by establishing relationships with departments across the firm.</li><li>Update and maintain institutional-related marketing and RFP materials.</li><li>Ensure the integrity of Bartlett’s brand, message, and client data in all presentation materials and client review pages.</li><li>Produce various periodic reports and handle special projects as needed.</li></ul>Strategy Implementation<ul><li>Support senior staff with day-to-day management of fixed income portfolios, including but not limited to matching portfolios to their associated models, monitoring cash levels, identifying new opportunities, and trade execution.</li><li>Develop and maintain customized portfolio analysis utilizing software including Morningstar Direct, Bloomberg, and other internal portfolio accounting and trading systems.</li><li>Maintain and regularly update an internal list of the fixed-income team’s favored investments with pricing, credit ratings, and related information.</li><li>Maintain broad knowledge of financial markets, current events, and economic trends, and evaluate their effects on Bartlett’s fixed income investment strategies.</li><li>Conduct research and analysis across the firm’s fixed-income holdings and effectively communicate findings to the broader team.</li></ul>Knowledge, Skills, and Abilities:<ul><li>Self-starter with a strong work ethic and ability to manage one’s work on a day-to-day basis without constant oversight, meet deadlines, and handle a high volume of work with accuracy.</li><li>Team-oriented approach with the ability to collaborate with Operations, Technology, Investment, and Marketing teams and develop effective relationships with colleagues.</li><li>The ability to communicate effectively with a wide range of people in a professional manner, both orally and written.</li><li>The ability to creatively enhance data through storytelling.</li><li>Strong organizational/process management skills with the ability to prioritize work.</li><li>Have a sense of urgency and keen attention to detail.</li><li>Analytical ability to troubleshoot and problem-solve.</li><li>High regard for confidentiality.</li><li>Show innovative and creative thinking.</li><li>Fundamental understanding of various asset classes including equity, fixed income, and alternatives.</li></ul>Position Requirements:<ul><li>Bachelor’s degree in a related field (Finance, Economics, or other business degree is preferred).</li><li>Knowledge of capital markets, investment strategies, and the institutional investment landscape.</li><li>Pursuing the CFA or having an interest in doing so, is highly desirable.</li><li>Some relevant experience within financial services or investment management is preferred.</li><li>Proficiency in Microsoft Office solutions, especially Outlook, Excel, PowerPoint, and Word.</li><li>Experience and familiarity working with Morningstar Direct, and Bloomberg is preferred.</li></ul>What we offer:<ul><li>A Top Workplace winning culture built on teamwork, integrity, and diligence.</li><li>Engaging work environment.</li><li>Opportunity for professional growth and development.</li><li>A hybrid work schedule.</li><li>Competitive salary and eligibility for annual firm profitability/individual performance incentive.</li><li>Comprehensive benefits package includes medical, dental, vision, life, and long-term disability insurance, flexible spending accounts and the items listed below.</li><li>401(k) plan with match and profit-sharing contribution.</li><li>Generous paid time off.</li><li>Paid parental and medical leave.</li></ul>Bartlett Wealth Management has 83 employees between its locations in Cincinnati, Chicago, and Louisville, and about $9.4 billion in assets under management as of December 31, 2024.#LI-Hybrid#J-18808-Ljbffr
