Overview:
Plan, implement, and maintain a volunteer program to meet the needs of families/caregivers, as well asregulatory requirements for the Medicare Conditions of Participation. Recruit, retain, and supervise volunteersinvolved in the hospice volunteer program.

Responsibilities:
<ul><li>Develop and deliver educational programs and materials for all volunteers.</li><li>Evaluate volunteer program services annually and provide recommendations for development, improvement, or expansion.</li><li>Provide information to individuals and community groups about hospice volunteer services.</li><li>Prepares monthly and annual service reports.</li><li>Acts as liaison between hospice staff and volunteers.</li><li>Participates in quality assurance and performance improvement activities as assigned.</li><li>Attend regular IDT meetings and complete IDT documentation.</li><li>Facilitates community awareness and support of UnityPoint at Home volunteer program.</li><li>Acts as liaison between hospice staff and volunteers.</li><li>Carry out other assignments as may be appropriate.</li><li>Participates in quality assurance and performance improvement activities as assigned.</li><li>Attend staff meetings and other meetings as assigned and appropriate.</li><li>Participate in orientation and training of new employees working in the hospice program.</li><li>Prepare service reports as requested by Hospice Administrator.</li><li>Assures volunteers and volunteer program meets Hospice Conditions of Participation.</li><li>Enter hospice volunteer frequency orders as needed.</li><li>Maintain a record-keeping system in SharePoint which includes services delivered and actual time involved.</li></ul>

Qualifications:
Education:<ul><li style="text-align: left;">Associates degree in business or related field or equivalent work experience.</li></ul>Experience:<ul><li style="text-align: left;">Minimum of two years’ experience working with patients/families/caregivers through death and dying issues</li></ul>Licensure/Certifications:<ul><li style="text-align: left;">Valid driver’s license when driving any vehicle for work-related reasons. </li><li style="text-align: left;">Valid Mandatory Reporter course completion by state(s) requirement</li></ul>#UPaHomeNursing
