Position Responsibilities

Responsibilities include but are not limited to:

️ Providing skilled nursing care and prescribed treatments️ Assessing patient conditions and implementing nursing care plans️ Monitoring and evaluating patient progress️ Coordinating care with physicians, therapists, nutritionists, and social workers️ Supervising LPNs, CNAs, HHAs, and subordinate staff as needed️ Identifying physical and emotional health concerns through assessment and observation️ Maintaining accurate nursing documentation and records️ Supporting patients and families with education and guidance

Qualifications

Active New York State Registered Nurse (RN) License REQUIRED Previous nursing home or long-term care experience preferred Strong clinical judgment and communication skills Ability to work independently and collaboratively within a healthcare team Compassionate and patient-focused approach to care
