Job Description

Mindify Wellness and Care is seeking an outgoing, compassionate, and relationship-focused Bilingual Care Coordinator to serve as a trusted community representative within our adult day care center partnerships throughout New Jersey.

In this role, you will help bridge the gap between individuals in need of behavioral health services and the care they deserve. As a vital member of the Mindify team, you will educate participants about ourmental health services, foster meaningful relationships with patients and caregivers, support continuity of care, assist with interpretation during therapy and psychiatric appointments when needed, and help connect individuals to appropriate behavioral health resources.

Work Location & Schedule

This is a part-time field-based position located within our partner adult day care centers throughout New Jersey.

The typical schedule is Monday through Friday, generally between 8:30 AM and 2:30 PM, with up to 30 hours per week. Daily schedules may vary based on center needs, patient engagement opportunities, outreach activities, and travel requirements. Some days may be shorter than six hours. Flexibility and adaptability are important in this community-based role.

Travel to assigned adult day care centers throughout New Jersey is required. Mileage reimbursement is available for eligible travel between multiple centers on the same day.

Key Responsibilities

Patient Engagement & Community Outreach

Build strong, trusting relationships with adult day care participants, caregivers, and center staff. Educate participants about Mindify's psychiatric, therapy, and behavioral health services. Serve as a welcoming and accessible resource for individuals seeking information about mental health care. Encourage participation in behavioral health services and promote overall wellness. Participate in outreach activities and help increase awareness of available behavioral health resources while reducing barriers to care.

Education & Presentations

Educate participants, families, caregivers, and center staff regarding available behavioral health services and treatment options. Answer questions about psychiatric and therapy services and assist individuals in understanding how to access care. Deliver engaging wellness discussions and informational presentations. Occasionally present to groups of 60 or more individuals when requested by leadership. Serve as a knowledgeable resource regarding behavioral health programs and support services.

Patient Advocacy & Care Coordination

Identify individuals who may benefit from behavioral health services and facilitate referrals. Assist patients in navigating available services and community resources. Support continuity of care by helping coordinate appointments and encouraging ongoing engagement in treatment. Advocate for patients by helping them understand treatment options and available support services. Serve as a liaison between patients, families, providers, adult day care center staff, and Mindify clinical teams. Assist with patient follow-up and engagement activities as needed. Perform additional assignments and duties as requested.

Relationship Management

Maintain strong relationships with adult day care center administrators, nursing staff, activities staff, social workers, and other key stakeholders. Foster collaborative partnerships that support patient care and organizational growth. Represent Mindify professionally within assigned centers and community partnerships. Identify opportunities to strengthen referral relationships and enhance services.

Translation & Interpretation Support

Provide Spanish-English interpretation and translation services for patients, families, and providers when needed. Assist with communication during therapy and psychiatric appointments. Support culturally responsive and accessible care for diverse populations.

Documentation & Compliance

Maintain accurate records of patient interactions, referrals, presentations, and outreach activities. Complete all required documentation in a timely and professional manner. Communicate regularly with leadership regarding referral activity, patient engagement, and growth opportunities. Maintain HIPAA compliance and patient confidentiality at all times. Perform additional patient advocacy, outreach, and support duties as assigned.

Qualifications

Required

Bilingual in English and Spanish. Excellent interpersonal, communication, and relationship-building skills. Strong public speaking and presentation abilities. Passion for working with seniors and diverse communities. Ability to build trust and create meaningful connections with patients and families. Comfortable engaging with individuals one-on-one as well as in group settings. Valid driver's license and reliable transportation. Willingness to travel throughout New Jersey. Ability to work independently and manage community-based responsibilities. Strong organizational and time-management skills. Proficiency with Microsoft Office and electronic systems.

Preferred

Previous experience in patient advocacy, patient services, healthcare, behavioral health, social services, senior services, community outreach, care coordination, customer service, or a related field. Experience working with older adults and underserved populations. Experience coordinating care or supporting patients in navigating healthcare services. Experience conducting presentations, educational workshops, or community outreach activities. Experience providing interpretation or translation services in healthcare settings. Experience working within adult day care centers or community-based healthcare programs.

Compensation

$24.00 - $30.00 per hour based on experience, qualifications, language abilities, and relevant healthcare or outreach experience.

This is a part-time position with up to 30 hours per week. Mileage reimbursement is available for eligible travel between centers on the same day.
