    Care Coordinator Bilingual Spanish

    The Care Coordinator is the key point of contact in managing services for the child. Moreover, the Care Coordinator oversees and coordinates access to a variety of services an individual needs to help prevent hospitalizations, stay healthy, and maintainstability. The Care Coordinator provides comprehensive Care Management and coordination of services to eligible Medicaid/Medicaid Managed Care children (ages 0-21) to develop and maintain a plan of access and use of services and supports necessary for their health. The Care Coordinator helps children and caregivers get the care they need to improve the child's health.

    Location: Hybrid- Peekskill Status: Full-time

    Essential Duties of Care Coordinator Bilingual Spanish

    Key duties of this position include, but are not limited to, the following:

    • Conduct eligibility screenings and Health Home enrollment.
    • Collaboratively work with enrollee to create a comprehensive person centered plan of care to meet the medical, behavioral health and social service needs to improve the individual's health and wellness.
    • Responsible for health promotion, education, and overall coordination of all aspects of an individual's care.
    • Offer family support, and referral/linkages to community, social, and other support services as needed.
    • Assist individual in keeping appointments and monitor service delivery.
    • Coordinate and arrange for the provision of medical, behavioral health and other relevant services
    • Accompany enrollees to appointments as needed
    • Conduct home visits as needed
    • Support enrollees in adherence to treatment recommendations from medical and behavioral health providers
    • Link enrollees to internal and external peer supports, support groups and self-care programs to increase knowledge about their disease(s), engagement and self-management capabilities, and to improve adherence to prescribed treatment
    • Promote evidence-based wellness and prevention by linking health home enrollees with resources for smoking cessation, diabetes, asthma, hypertension, self-help recovery resources, and other services based on individual needs and preferences
    • Travel as required to community based meetings, medical/behavioral health provider sites and meetings with Health Home enrollees
    • Maintain electronic medical records in compliance with ANDRUS and DOH standards
    • Supports overall organization strategic plan of the organization

    Essential Qualifications of Care Coordinator Bilingual Spanish

    Key qualifications include:

    Bachelor's Degree in Human Services and/or related field.

    Bilingual Spanish speaker/read/write

    Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare and/or other systems as well as those receiving preventive services.

    Excellent writing, communication, and organizational skills.

    A passion for serving children and families with special needs.

    Ability to access and utilize ADP app on smartphone or other device for access to agency programs.

    Physical/Travel Requirements of Care Coordinator Bilingual Spanish

    Valid NYS Driver's License

    Ability to travel to community-based agency offices, participant's communities, and homes within assigned catchment area

    The Care Coordinator must be able to drive to external locations and work flexible hours as required to meet program needs with some evenings and/or weekends as identified.
