    Care Coordinator I (EAP)

    The Elder Abuse Prevention Program is currently seeking a Care Coordinator/Social Worker to serve the Long Beach regional office and its surrounding territory (SPA 8). The Care Coordinator will provide comprehensive care management services to older adults and disabled residents, which include a comprehensive assessment of the clients, development of a written care plan, arrangement and coordination of services, and client follow-up to monitor the effectiveness of the services arranged and assess the need for additional services.

    WISE & Healthy Aging's Elder Abuse Prevention Program provides strength-based and person-centered in-home case management services to older adults and adults with disabilities residing in SPA 8 (Long Beach/South Bay and surrounding areas). The Care Coordinator is responsible for providing efficient, thorough, and effective care management services, which include a comprehensive assessment of the clients, development of a written Care Plan with measurable outcomes, arrangement and coordination of services, and client follow-up to monitor the effectiveness of the services arranged and assess the need for additional services. The position may also lead support groups and/or presentations.

    Primary responsibilities include:

    • Conduct client intakes with potential clients to identify needs and determine client eligibility. Perform an in-home assessment within 14 days of completing the client intake process.
    • Field travel to clients' homes and other sites as needed, up to 30-70% of the time.
    • Conduct client intake screening and in-home assessments using program-specific forms to identify client needs and confirm eligibility.
    • Conduct a comprehensive in-home assessment and write a biopsychosocial assessment. Complete paper documentation, including progress notes and billing, for all CM protocols.
    • Develop a written Care Plan with measurable outcomes to address and implement an agreed-upon action plan for client needs and complete it based on contract requirements.
    • Identify appropriate resources and mobilize them to address client needs and resolve problems.
    • Arrange and coordinate services on behalf of the client.
    • Maintain contact with client to monitor the effectiveness of the service arranged for client and assess the need for additional services.
    • Facilitate existing support groups as well as start up new groups and/or workshops.
    • Assist with material outreach and community outreach as assigned.
    • Write accurate, thorough, and specific client assessments, letters, emails, and other communications. Complete timely, accurate, and thorough documentation for client files.
    • Ensure service recording, including accurate billing, is entered into software systems every week as required.
    • Prepare for and participate in staff meetings.
    • Assist with other related CM projects as assigned.

    Essential job functions include:

    • Ability to perform biopsychosocial assessments of clients using appropriate techniques.
    • Ability to document client information and maintain client boundaries and confidentiality.
    • Ability to visit clients' homes with the use of a personal vehicle or other means of personal transportation.
    • Ability to work agency office hours 8:30 am – 5:00 pm.
    • Ability to effectively communicate expectations to team members.
    • Strong interpersonal, verbal, and written communication skills.
    • Ability to demonstrate good judgment and decision-making skills.
    • Ability to multitask and prioritize work with competing and conflicting deadlines.
    • Proficient knowledge of Microsoft Office software programs and Excel.

    Other responsibilities include:

    • Represent the agency at community outreach events. Assist with presentations and workshops on program services.

    Due to changing program demands, funding source expectations/or changes in agency policy, as assigned, staff may be asked to perform other duties.

    Position qualifications include:

    • Master's degree in social work, gerontology, human services, or a related field required.
    • Up to (2) years of related experience (voluntary or paid) in gerontology work, health care, or social services strongly preferred.
    • Demonstrated experience in performing direct service in social services or a related field. Prior responsibilities in handling case management, client advocacy, or therapy involving vulnerable populations, particularly older adults.
    • Fluency in English (bilingual in Spanish strongly preferred).
    • Previous experience working with an ethnically and economically diverse population.
    • Ability to travel via personal vehicle throughout Los Angeles County/City. Valid driver's license and up-to-date automobile insurance are required throughout employment.
    • Ability to communicate effectively in writing and orally.
    • Excellent organizational and telephone skills.
    • Ability to interface/relate well with all levels of staff and with the public.
    • Livescan background and TB Test clearance required.

    Program funding is allocated on a fiscal year basis, July 1st through June 30th each year. Termination of all or part of these funds at any time during the funding period may affect the agency's continued ability to fund this position.

    The employee may be expected to enter premises during field visits that may be environmentally challenging, including but not limited to environments with cigarette smoke, bed bugs, dust, and other allergens.

    This position is based at the Long Beach office of WISE & Healthy Aging.
