A nonprofit healthcare center 12 miles north of Boston that provides comprehensive medical and supportive services for adults aged 55 and older through the PACE (Program of All-Inclusive Care for the Elderly) model is adding a Geriatrician. The organization's mission is to help seniors remain safely in their homes and communities for as long as possible by offering coordinated care that includes primary and specialty medical services, rehabilitation, social programs, meals, and transportation.
About the Opportunity:
This position will focus on care in our skilled nursing facilities
Monday through Friday, 8 am-4 pm
Average daily patient census is 8-15
Provide complete and highly personalized care to help seniors live independently, safely, and comfortably in their homes for as long as possible
Deliver team-based care that includes physicians, nurses, occupational and physical therapists, social workers, behavioral health specialists, and CNAs,
Market rate salary plus full benefits, including health, dental, vision insurance, generous PTO, and retirement plan
An ideal candidate should have 5+ years of clinical practice experience, preferably in a value-based care organization, knowledge of HEC risk adjustment, and experience working in an IDT environment.
