Services are provided in home health settings and focus on helping patients safely perform activities of daily living (ADLs), improve upper extremity function, and adapt to physical limitations.ResponsibilitiesEvaluate patients and develop individualized treatment plans based on physician orders and clinical findingsAssess patients' ability to perform activities of daily living, including dressing, bathing, grooming, meal preparation, and household tasksDevelop therapeutic interventions to improve functional mobility, coordination, strength, cognition, and independenceRecommend adaptive equipment, assistive devices, and environmental modifications to promote safety and independenceEducate patients, family members, and caregivers on therapeutic techniques, home safety, and proper use of adaptive equipmentIdentify safety concerns and fall risks within the home environment and recommend appropriate interventionsMonitor patient progress and adjust treatment plans as needed to achieve established goalsCollaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate careCommunicate changes in patient condition and treatment response to the physician and Director of NursingComplete accurate clinical documentation, progress reports, and discharge summaries in a timely mannerTreat clients, families, caregivers, and team members with professionalism, courtesy, and respectQualificationsMaster's Degree or Doctorate in Occupational Therapy from an accredited occupational therapy programCurrent Occupational Therapist license in the state where services are providedSuccessful completion of all state-required licensing requirementsMinimum of two (2) years of experience as an Occupational TherapistHome health or community-based experience preferredStrong clinical assessment, communication, and documentation skillsCommitment to maintaining HIPAA compliance and patient confidentialityJoin Our TeamIf you're passionate about helping patients maintain independence and improve their quality of life while enjoying a flexible work schedule, we'd love to hear from you.
