Job Summary
Queen City Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide direct patient care in a hospice setting. This role focuses on delivering high-quality, patient-centered care that supports comfort, dignity, and quality of life for patients and their families. The ideal candidate has strong clinical skills, excellent communication abilities, and a passion for end-of-life care.
Key Responsibilities
- Provide direct patient care in accordance with the State Nurse Practice Act
- Deliver nursing care based on comprehensive patient assessments and established plan of care
- Accurately document patient care and services in a timely manner
- Educate patients and families on diagnosis, care plans, and symptom management
- Coordinate hospice services with the interdisciplinary care team
- Communicate patient needs, status changes, and care updates effectively
- Provide pain and symptom management interventions
- Monitor and evaluate patient responses to treatments and medications
Required Qualifications
- Graduate of an accredited Practical Nursing (LPN/LVN) or Vocational Nursing program
- Current Licensed Practical Nurse (LPN/LVN) license in the state of practice
- Minimum 3 years of nursing experience
- Current CPR certification
- Strong verbal and written communication skills
- Excellent organizational and time management skills
- Valid driver's license and reliable, insured vehicle
Preferred Qualifications
- Experience in hospice, home health, community health, or medical-surgical nursing
- Understanding of hospice philosophy and end-of-life care principles
