Job Overview

We are seeking a dedicated Physical Therapist (PT) to provide high-quality rehabilitative care. This role focuses on helping patients regain mobility, strength, and independence in a supportive skilled nursing environment.

Key Responsibilities

Evaluate and develop individualized treatment plans

Provide therapy to improve mobility, strength, and function

Monitor patient progress and adjust care plans accordingly

Collaborate with interdisciplinary teams

Educate patients and families on recovery goals

Maintain accurate documentation

Requirements

Active Physical Therapist (PT) license (or eligibility)

Experience in skilled nursing or rehab preferred

Strong communication and patient care skills
