Shift Details:

• 
  Monday-Friday 8:00am-4:30pm
  

Social Worker  Duties & Responsibilities:

• 
  Assessing individuals, developing individual care plans and determining necessary resources.
  
• 
  Advocating for and assisting individuals and families in accessing and obtaining services.
  
• 
  Contacting service providers and attending interdisciplinary conferences to determine appropriate placement and referrals.
  
• 
  Monitoring individual rehabilitation goals and progress.
  
• 
  Providing crisis intervention services.
  

Minimum Qualifications 

• 
  Current, valid, and unrestricted NY license/certification in good standing, as required for the position.
  
• 
  Minimum of one (1) year  related experience
  
• 
  Bachelor's in Social Work degree, working under the supervision of a licensed master social worker (LMSW) or licensed clinical social worker or New York State LCSW or LCSWs.
  
• 
   Valid drivers license.
  
• 
  Additional certifications may be required.
  
• 
  Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
  
• 
  Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
