Reports to:Director of Pupil Personnel Services
Performance Responsibilities:
Provide direct physical therapy as prescribed by student's IEP
Evaluate referred students regarding physical needs.
Write physical therapy objectives for student's physical therapy needs in clear concise measurable language.
Provide consultation and modeling to classroom staff regarding physical therapy interventions.
Develop and demonstrate home programs when necessary. Instruct parents in follow through programs when appropriate.
Maintain regular and ongoing communication with teacher(s) and other pertinent school staff to insure optimum physical therapy.
Maintain ongoing data-based records on student's progress.
Participate as a member of the child evaluation team for assigned students.
Attend and participate in staff meetings.
10.Assume responsibility for providing teachers with written progress information for inclusion in student's quarterly progress report.
