24 - 32 hr RN Case Manager - VNA Home Health Care - Arlington & Belmont Area

VNA Care is hiring a Case Manager RN to join our team for 24-32 hours per week. This position will cover patient visits in the Arlington and Belmont areas.

The ideal candidate will be dedicated to providing high-quality, compassionate care to patients in their homes, ensuring they receive the medical attention, support, and services they need to thrive.

Salary range: $86,000 - $129,00 for a full-time schedule, would need to adjust for a part time schedule.

Schedules:Work three to five days, 8a-2p or 8a-4:30p, Monday - Friday

Qualifications:

• Must hold a valid MA RN License in good standing
• Minimum of 1 year of med/surg experience; acute care experience is beneficial
• Ability to work with compassion, organization, and excellent multitasking skills
• Willingness to travel within a designated territory
• Must have Home Health Care experience

Responsibilities

• Conduct home visits to assess patient needs and provide necessary care
• Utilize Case Management, Oasis E documentation, Time Management, and EMR system, Home Care Home Base
• Collaborate with patients, families, and interdisciplinary team members to develop and implement care plans
• Ensure accurate documentation of patient progress and treatment plans
