About the Role:
Join PERSONALIZED CARE FOR ALL LLC as a 245D Coordinator in St. Paul, MN, where you will play a pivotal role in enhancing the quality of life for individuals with disabilities. This exciting position offers the opportunity to lead and coordinate essential services, ensuring that our clients receive personalized care tailored to their unique needs.



Responsibilities:

• Develop and implement individualized service plans in compliance with 245D regulations.
• Coordinate care services and resources for clients, ensuring seamless delivery of support.
• Conduct regular assessments and evaluations to monitor client progress and adjust plans as necessary.
• Facilitate communication between clients, families, and service providers to enhance collaboration.
• Maintain accurate documentation of client services and progress in accordance with state guidelines.
• Provide training and support to staff on 245D standards and best practices.
• Advocate for clients' rights and needs within the community and service systems.
• Stay updated on industry trends and regulatory changes to ensure compliance and quality service delivery.

Requirements:

• Bachelor’s degree in social work, human services, or a related field preferred.
• Minimum of 2 years of experience in coordinating services for individuals with disabilities.
• Knowledge of 245D regulations and best practices in service delivery.
• Strong organizational and communication skills, both verbal and written.
• Ability to work collaboratively with diverse teams and clients.
• Current Minnesota driver’s license and reliable transportation.
• Compassionate demeanor with a commitment to client advocacy and empowerment.
• Certification in CPR and First Aid is a plus.
