This role provides an opportunity to support individuals experiencing serious mental illness and co-occurring conditions as they work toward housing stability, recovery, and community integration.As a 44 North Case Manager, you will play a vital role in helping residents maintain stable housing, access community resources, develop life skills, and achieve their self-identified goals. Our team utilizes Housing First, trauma-informed, strengths-based, and recovery-oriented approaches to help residents build stability, independence, and long-term success.Residents receive individualized support through case management, life skills development, community integration activities, and connections to healthcare, employment, education, and other essential resources.What You'll DoAs a 44 North Case Manager, you'll provide comprehensive case management and supportive services designed to help residents thrive in the community.Case Management ServicesCollaborate with residents to identify needs and develop individualized service plansMeet regularly with residents to review goals, progress, and evolving needsSupport residents in maintaining housing stability and complying with housing requirementsAssist residents with housing recertifications and required documentationConnect residents to healthcare, employment, education, benefits, and community resourcesAssist residents in obtaining and maintaining eligibility for Medicaid, SNAP, and other benefitsProvide client advocacy and support navigation of complex systemsUtilize assertive outreach and engagement strategies to encourage participation in servicesLife Skills & Community IntegrationDevelop and facilitate life skills groupsCoordinate transportation for appointments, community activities, and special eventsSupport resident participation in activities that promote independence and community engagementUtilize motivational interviewing techniques to support self-directed recovery goalsApply Housing First, trauma-informed, and strengths-based approaches in all interactionsDocumentation & ComplianceMaintain accurate and timely documentation in KIPU, HMIS, Azure, and other required systemsComplete case notes, assessments, care plans, and related documentation within required timeframesEnsure compliance with HIPAA, 42 CFR Part 2, and organizational policiesComplete HRSS documentation, billing requirements, and fund requests as applicableMaintain organizational productivity and documentation standardsTeam CollaborationCollaborate effectively with clinical, housing, medical, and community partnersParticipate in team meetings, supervision, and required trainingsAssist the Program Manager with administrative and program support tasks as neededServe as a positive role model through professional, recovery-oriented interactionsWhy You'll Love This RoleBuild meaningful relationships with residents and support their recovery journeyHelp individuals maintain stable housing and improve quality of lifeWork within a supportive multidisciplinary teamUtilize evidence-based engagement strategies including Motivational InterviewingMake a direct impact on reducing homelessness and improving community integrationJoin a mission-driven organization dedicated to changing livesQualificationsPreferredBachelor's Degree in a human services-related fieldActive Utah Social Service Worker (SSW) licenseRequiredCase management experienceStrong interpersonal, communication, and problem-solving skillsAbility to work effectively with individuals experiencing serious mental illness and co-occurring disordersStrong organizational skills and attention to detailAbility to document services accurately and professionallyProficiency with Microsoft Office and electronic documentation systemsReliable attendance and punctualityPreferred ExperienceExperience working with individuals experiencing serious mental illnessExperience with substance use disordersExperience working with individuals experiencing homelessnessFamiliarity with Housing First and recovery-oriented models of careExperience facilitating groups and community-based servicesFamiliarity with KIPU and HMIS systemsWhat We're Looking ForThe ideal candidate is:Recovery-oriented and strengths-basedTrauma-informed and person-centeredSkilled at building rapport with individuals who have complex needsOrganized and detail-orientedCollaborative and team-focusedComfortable working both independently and as part of a multidisciplinary teamPassionate about helping individuals achieve stability, independence, and meaningful recoveryAt First Step House, every day is an opportunity to make a lasting impact.
