988 Tribal Response Case Manager

POSITION SUMMARY: Responsible for providing coordinated case management services for individuals served through Comanche Nation Prevention & Recovery programs, including MAT PDOA, Tribal Opioid Response (TOR), Suicide Prevention and Intervention Program (SPIP Native Spirit), IHS initiatives, and the 988 Tribal Response Program. The Case Manager supports individuals experiencing substance use disorders, mental health challenges, suicidal ideation, or crisis transitions by coordinating assessments, referrals, service planning, follow up, and continuity of care in compliance with funding requirements and Tribal standards.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Maintain confidentiality at all times.
• Maintain a positive, professional, and courteous demeanor at all times.
• Demonstrate calm, competent responses during emergencies or crises.
• Conduct intake, screening, and needs assessments for program participants.
• Develop, implement, and monitor individualized service and care plans.
• Coordinate referrals to behavioral health, MAT services, crisis response, recovery supports, and community resources.
• Support individuals transitioning from crisis lines, emergency departments, inpatient services, detention, or residential treatment.
• Coordinate follow up and engagement activities for individuals served through the 988 Tribal Response Program.
• Collaborate with Outpatient Director, Project Managers, PRSS, Outreach staff, clinical providers, and community partners.
• Ensure continuity of care across prevention, treatment, recovery, and postvention services.
• Maintain accurate, timely, and compliant documentation in electronic and paper records.
• Ensure compliance with HIPAA, 42 CFR Part 2, and Tribal confidentiality requirements.
• Participate in required trainings, supervision, team meetings, and quality improvement activities.
• Perform general clerical and documentation support tasks related to case management services.
• Perform other duties as assigned.

SUPERVISORY RESPONSIBILITIES AND REPORTING:

• Reports to the Project Manager, under the supervision of the Outpatient Director.
• This position does not have a supervisory authority.
• Project Managers provide grant oversight only.

PHYSICAL DEMANDS:

Ability to perform office and community based work, sit or stand for extended periods.

Travel to community locations.

Work Schedule: Monday through Friday, 8 am to 5pm. Some evenings and/or weekends.

WORK ENVIRONMENT: Professional and confidential behavioral health work environment.

QUALIFICATIONS:

• High school diploma or equivalent required; Associate or Bachelor's degree in social work, human services, psychology, public health, or related field preferred.
• Minimum one (1) year of experience in case management, behavioral health, substance use services, crisis response, or related field.
• Knowledge of substance use disorders, mental health services, crisis systems, and community resources.
• Experience working with Tribal or Indigenous communities preferred.
• Strong organizational, documentation, and communication skills.
• Proficiency in Microsoft Office and electronic record keeping systems.
• Valid Oklahoma driver's license and insurable under the Comanche Nation motor vehicle policy.
• Must successfully complete background check and drug screening.
