Overview
The Care Coordination Supervisor will be responsible for the day-to-day operations of our Care Coordination Services for the respective service area and ensuring seamless team management and development, program delivery, and quality improvement and evaluation.

Organizational Relationships
Team Management and Development
Develop, implement, monitor and evaluate program delivery improvement.
Develop, implement, monitor and revise policies and procedures related to community based programs as required by governing bodies.
Provide initial and ongoing training on new policies to all staff.
Coordinates with the Director and direct service staff to ensure stakeholders involvement in program policy development.
Qualifications
Holds a minimum of a 2-year degree in a Humans Services related field.
At least five years of experience in Human Services related field.
Valid driver's license when driving any vehicle for work-related reasons.
Broad knowledge of licensure, accreditation, governing body standards for community based programs.
Strong leadership skills and experience in facilitating interdisciplinary collaboration.
Strong organizational skills.
Excellent verbal and written communication skills with exceptional attention to details.
Demonstrated success developing and evaluating program models, and selecting and successfully operationalizing innovative programs.
Proficient in using technology as a management reporting tool and experience working with information technology staff to develop and implement program evaluation systems.
Strong project management skills managing complex, multifaceted projects resulting in measurable successes and program growth.
Strong understanding of human resources, employee performance improvement plans, and corrective action policies.
