Assistant Care Coordinator/Care Lead

Champlain Valley Senior Community is seeking an experienced, dependable, and compassionate Assistant Care Coordinator (ACC)/Care Lead to help supervise resident-care operations and support our care team in providing safe, respectful, person-centered services.

Responsibilities:

• Provide shift leadership and direction to HHAs, RCAs, PCAs, and medication technicians

• Ensure resident assignments are completed accurately and on time

• Monitor resident care, safety, appearance, comfort, and overall well-being

• Respond promptly to call bells, resident concerns, and changes in condition

• Report changes in residents’ physical, emotional, or behavioral condition to the appropriate nurse or supervisor

• Assist residents with personal care, mobility, transfers, toileting, meals, hydration, and other activities of daily living when needed

• Confirm that required documentation is completed before the end of each shift

• Review assignment sheets and ensure required resident checks and toileting schedules are followed

• Help coordinate shift coverage and immediately report staffing concerns

• Support new employees during orientation and on-the-job training

• Promote teamwork, accountability, respectful communication, and professional conduct

• Follow residents’ individualized service plans, and all facility policies

• Assist with emergency procedures and incident reporting as required

• Maintain residents’ dignity, privacy, safety, and independence

Qualifications:

• Previous experience in assisted living, memory care, long-term care, or senior care required

• Previous supervisory or team-lead experience preferred

• Current HHA or PCA certification preferred

• Medication Technician experience or certification is preferred

• Strong leadership, organization, communication, and problem-solving skills

• Ability to remain calm and provide direction during emergencies or challenging situations

• Ability to perform the physical duties of the position safely

• Must successfully complete all required background checks, health requirements, training, and employment documentation

Schedule:

Full-time availability is preferred. Evening, weekend, and holiday availability may be required based on facility needs.
