Job Summary

Facilitate the admission and /or transfer of patients to Charleston Area Medical Center based on capacity and capability. Coordinate and assign internal and external patient placement through the Transfer Center via Cerner computer applications.

Responsibilities

  • Receive request from physician for transfer of patient to CAMC; determine the availability of requested placement; contact CAMC physician for acceptance of patient; coordinate three-way(or more) conversation between referring provider and CAMC physicians and/or provider; patient intake referral via Care Aware Transfer Center Solution; assign patients to beds via Capacity Management Cerner application; connect referring providers with Med Base for assist with transport when needed; acquire all pre-registration information; advise referring hospital/physician and/or provider of required medical information for transfer; create FIN and enter planned PSO for all direct admissions.
  • Maintain responsibility for taking all Physician Access Line calls along with management of internal customer calls.
  • Document compliance with standards established to monitor referral activities to medical staff and administration.
  • Proficient computer skills including but not limited to Microsoft programs.
  • Report daily the AM census snapshots regarding occupancy and bed availability for all four hospitals via email to medical staff and administration.
  • Make recommendations to improve referral process.
  • Attend daily bed meetings to proactively deal with patient placement issues along with collaborating with Patient Flow Coordinators and/or Night Supervisors.
  • Collaborate with patient care management staff regarding bed availability and staffing issues in order to open or close beds in Capacity Management for all patient flow.
  • Communicate information on hospital occupancy and bed availability to patient care management staff and administration when needed.
  • Arrange for internal transfers in accordance with physicians' orders, bed management guidelines, and unit criteria.

Job

  • Monitor compliance with internal policies and procedures related to bed utilization including but not limited to yearly EMTALA education.
  • Make recommendations to improve patient flow through critical thinking skills and collaboration with patient care management staff, Patient Flow Coordinators, Night Supervisors and Administration.

The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department's identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients.

Education

  • Associate's Degree (Required) Experience: 3 Years Related Clinical Nursing Experience in Acute Care Substitution: May substitute Formal Nursing Program for the Associate Degree
  • Bachelor's Degree

Status: 72 hrs per pay
