Position Overview
We are seeking an organized and dynamic professional to serve as the ACT Team Leader. In this role, you will oversee the daily operations of a multidisciplinary ACT team, ensuring the delivery of high-quality, recovery-oriented services to individuals with severe and persistent mental illness. The Team Leader provides clinical and administrative supervision, fosters team collaboration, and ensures adherence to ACT model fidelity and regulatory standards.

Key Responsibilities

  • Provide leadership, supervision, and support to ACT team members, including clinicians, nurses, case managers, and peer specialists
  • Oversee daily team operations, including scheduling, service delivery, and crisis response
  • Ensure the development, implementation, and monitoring of individualized treatment and recovery plans
  • Promote a person-centered, strengths-based approach to care
  • Facilitate daily team meetings and case conferences
  • Monitor program performance and ensure compliance with agency policies and ACT fidelity standards
  • Coordinate care with external providers, community partners, and stakeholders
  • Provide at least 50% of your time in direct clinical services and crisis intervention as needed
  • Support staff development through coaching, training, and performance evaluations
  • Maintain accurate documentation and reporting in accordance with regulatory requirements
  • Organize and provide 24/7 on-call coverage
  • Other duties as assigned

Qualifications

  • Master's degree in Social Work, Counseling, or Marriage & Family Therapy Required
  • Current licensure in NYS (e.g., LCSW, LMSW, LMFT, LMHC) preferred
  • Minimum of 2 years of experience working with individuals with serious mental illness
  • Knowledge of the ACT model, recovery-oriented practices, and community-based behavioral health services
  • Strong leadership, organizational, and interpersonal skills
  • Ability to manage multiple priorities in a fast-paced environment

Preferred Qualifications

  • Experience working within an ACT team or other field-based team 
  • Bilingual skills are a plus
  • Familiarity with electronic health records (EHR) systems
