Acute Care Nurse Practitioner (Outpatient Primary Care)
Setting: Outpatient clinic(s) within a local market (may float between nearby sites)

Overview

An established, mission-driven healthcare organization is seeking an Acute Care Nurse Practitioner to deliver outpatient primary care with an emphasis on same-day/acute needs for an older adult (primarily Medicare-aged) patient population. This role partners closely with a multidisciplinary care team to deliver timely, high-quality care and an excellent patient experience.

Key Responsibilities

• Provide outpatient primary care and consultative services with a focus on acute needs (walk-ins, sick visits, hospital follow-ups, urgent evaluations).
• Document patient care thoroughly and accurately (encounter notes, diagnoses, treatment plans, and continuity documentation).
• Coordinate and communicate plans of care with patients assigned Primary Care Providers to ensure continuity.
• Collaborate with RNs, Medical Assistants, and operations/clinical leadership to identify patients needing outreach or acute support and deliver appropriate services.
• Participate in clinical meetings (quality improvement, outcomes review, peer review, hospitalized patient reviews as applicable).
• Support community engagement events as needed (outreach/education/relationship building).
• Participate in call coverage on nights/weekends/holidays as scheduled (as applicable to the market/team coverage model).

Qualifications

• Active, unrestricted RN/APRN license in the state of employment (in good standing / non-probationary).
• Minimum 1 year relevant experience in primary care (e.g., internal medicine, family medicine, geriatrics).
• Current BLS and ACLS certifications.
• Comfortable managing complex, medically underserved older adult populations with comorbidities.
• Strong clinical assessment/diagnostic skills and patient-centered communication.
• Team-oriented, collaborative, and committed to a high-quality patient experience.
• Ability to travel locally between nearby clinic sites within the market.
