The Acute Care Physical Therapist provides physical therapy evaluation and treatment to patients with musculoskeletal and/or neurological impairments. The Physical Therapist is also responsible for the monitoring and documentation of patient status, treatment, and response.
Responsibilities:
- Performs physical therapy evaluations and develops a plan of care that is appropriate for the diagnosis and age of patient.
- Performs physical therapy patient treatment as per the plan of care and modifies treatment plan based on ongoing assessment of patient condition.
- Demonstrates an understanding of the operation and indications for commonly used modalities and equipment.
- Documents evaluations, daily notes, progress notes, and discharge notes in a timely manner; therapy charges are supported by this documentation.
- Communicates and documents necessary information appropriately with patients, families, physicians, and interdisciplinary team members.
Requirements:
- BLS Certification
- Valid Wisconsin Driver’s License
- Current WI Physical Therapy License
