Job Summary:

Care New England, a fully integrated health system serving residents throughout Rhode Island is seeking a Nurse Practitioner or Physician Assistant to work as a member of their Integra at Home program. Integra at Home provides 24/7 clinical oversight of a select group of high-risk patients through nurse care management, in home NP/PA visits, community paramedicine, and remote patient monitoring, all with the aim of preventing unnecessary emergency department visits or hospital admission.

  • This role utilizes a hybrid care model combining in-person visits and telehealth
  • Average Patient Age: 60+
  • Full time - 5 ( 8hr days) or 4 (10hr day) work week with rotational paid call

Job Responsibilities:

  1. Provide evaluation and treatment of acute and acute-on-chronic medical conditions in the home or via MIH (Mobile Integrated Health) supported virtual visits.
  2. Manage common acute diagnoses including COPD and CHF exacerbations, skin and soft tissue infections, pneumonia, viral upper respiratory infections, and urinary tract infections.
  3. Deliver intensive in-home diagnostic and treatment pathways, including laboratory testing, imaging, respiratory therapies, and IV medications.
  4. Recognize clinical deterioration and escalate care to the emergency department when patients cannot be safely managed in the home.
  5. Work closely with a multidisciplinary team including case managers, nurses, physicians, social workers, and pharmacy.
  6. Provide early post-discharge and high-risk follow-up visits for patients recently discharged from the hospital or skilled nursing facility to reduce readmissions.
  7. Provide focused patient and caregiver education related to acute and chronic conditions.
  8. Participate in interdisciplinary case reviews and quality improvement initiatives focused on acute care outcomes and hospitalization avoidance.

Requirements:

  • Graduate of an accredited Physician Assistant or Nurse Practitioner program
  • Certified by NCCPA or ANCC or AANPCB
  • Active Rhode Island PA, CSR, and DEA license or eligible.
