Acute Order Coordinator

To serve as the pharmacy’s front-line coordinator for time-sensitive medication orders, ensuring acute and urgent prescriptions are identified early, reviewed promptly, and advanced through a defined process so that residents receive critical medications as quickly as possible. This role is central to ResidentRx’s proactive, pharmacy-driven model for medication access.

Responsibilities

  • Monitor incoming medication orders throughout the day to identify acute and time-sensitive prescriptions, including new admits, antibiotics, pain medications, insulins, and steroids.
  • Initiate the acute order process immediately upon identifying a qualifying order, rather than allowing it to wait in the general queue.
  • Review acute orders for clinical completeness, flagging missing information, potential interactions, or prior authorization requirements close to the time the order is received.
  • Coordinate with prescribers, community staff, and pharmacists to verify orders, resolve issues, and obtain any required authorizations.
  • Arrange backup fill and delivery logistics for acute medications, targeting same-day delivery to the community when clinically appropriate.
  • Communicate proactively with community teams regarding order status, expected delivery timing, and any delays or issues that arise.
  • Maintain accurate documentation of all acute order activity, including timestamps, actions taken, and outcomes, to support operational reporting and quality improvement.
  • Track and escalate orders that encounter barriers such as stock-outs, prior authorization holds, or prescriber response delays, working to resolve them before end of day.
  • Collaborate with the pharmacy team to distinguish between acute orders requiring immediate intervention and routine orders that follow the standard nightly UPS delivery process.
  • Support the onboarding of new communities by helping establish acute order expectations and communication workflows.
  • Participate in the Quality Management Program by reporting quality-related concerns through appropriate channels. Reporting events related to patient safety is mandatory.
  • Perform all other duties as assigned by the Pharmacy Manager or operations leadership.

Core Competencies

  • Proactive Mindset – Anticipates problems before they escalate. Does not wait to be told something is urgent; identifies it, acts on it, and follows through.
  • Clear Communication – Communicates directly and effectively with prescribers, community staff, and pharmacy team members. Keeps people informed without being asked.
  • Problem Solving – Thinks through barriers quickly and works toward solutions. When a prior auth is needed, a medication is out of stock, or a prescriber is unresponsive, this person finds a path forward.
  • Team Orientation – Works collaboratively across the pharmacy team, supports colleagues during high-volume periods, and contributes to a positive, reliable team environment.
  • Urgency and Follow-Through – Operates with a sense of urgency appropriate to the clinical situation. Tracks open items and does not let things fall through the cracks.
  • Adaptability – Adjusts to shifting priorities throughout the day as new orders come in and situations change. Comfortable working in a fast-paced environment where no two days look the same.
  • Attention to Detail – Accurately documents actions, catches discrepancies in orders, and maintains a high standard of precision even under time pressure.

Knowledge, Skills, and Abilities

  • High school diploma or equivalent (required).
  • Valid state pharmacy technician registration (where required).
  • National Pharmacy Technician Certification (CPhT) preferred.
  • 1–2 years of pharmacy experience, preferably in long-term care.
  • Experience with pharmacy medication order input and processing.
  • Understanding of prior authorization processes, insurance adjudication, and third-party plan parameters.
  • Familiarity with acute medication categories (antibiotics, controlled substances, insulins, steroids) and their clinical urgency.
  • Strong organizational skills with the ability to prioritize and manage multiple time-sensitive tasks simultaneously.
  • Excellent communication skills, both written and verbal, with the ability to work effectively with prescribers, community staff, and pharmacy team members.
  • Ability to remain calm and focused under pressure, particularly during high-volume order periods.
  • Proficiency in pharmacy software systems, including order entry, tracking, and documentation tools.
  • Detail-oriented with strong follow-through on open items and escalations.
  • Ability to maintain patient confidentiality and comply with HIPAA requirements.
  • Self-motivated with the ability to work independently while keeping the broader team informed.
  • Dependable attendance and punctuality to ensure timely patient care.
  • Willingness to cross-train and adapt to evolving workflows as the pharmacy operation scales.
  • Flexibility to work evenings, weekends, and holidays as necessary.
