Job Overview
Fairview is hiring a 0.8 FTE Inpatient Oncology Social Worker to join our team at University of Minnesota Medical Center in Minneapolis, Minnesota. This benefit-eligible position is scheduled for four days per week and includes an every sixth weekend rotation and one holiday per calendar year. The acute social worker works on-site with the interdisciplinary teams including providers, physical therapists, nurses, dieticians, chaplains and pharmacists to assist patients and families in navigating through their hospitalization. The acute social worker completes psychosocial assessments, care planning/ progression and creates comprehensive discharge plans from the hospital. The social worker is patient facing, responds to crisis situations, manages patient and family expectations, creates solutions to identified discharge barriers and aims to reduce length of stay and prevent avoidable hospital readmissions.

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
