Description:• The Addiction Counselor position is a full-time role located in Richmond Hill, NY with shifts available either Sun-Thu or Tue-Sat from 01:00pm-09:00pm.
• This role involves working onsite and focuses on providing support to patients with substance use disorders using a trauma-informed approach.
• Key responsibilities include individual counseling, group facilitation, and coordination of care.
Requirements:• Provide individual counseling and facilitate therapeutic groups.
• Conduct assessments, screenings, and intakes.
• Use a trauma-informed approach to support patient recovery.
• Coordinate care with internal and external teams.
• Maintain accurate and up-to-date documentation.
• Participate in crisis intervention when needed.
• Assist in referrals and scheduling for appropriate care.
Qualifications:• Experience in managing a caseload of patients with substance use disorders.
• Ability to develop treatment plans with clinical leadership.
• Strong collaboration skills with treatment team members.
• Experience in clinical consultation with referral resources and family members.
• Skilled in engaging patients in decision-making processes.
• Proficient in collecting and reviewing referral information.
• Experience in monitoring patient access and involvement with treatment.
