Administrative Coordinator (MA or LPN)

TANDEM365 is seeking an organized, self-driven, and compassionate healthcare professional to serve as a key member of our care team.This position offers the opportunity to work as an essential part of an interdisciplinary health care team. This position includes participation in a weekend schedule to support Participant enrollment and operational needs.

Our services help them remain in their homes and manage their conditions more effectively. Our model provides a patient-centered method of care delivery to reduce gaps in care and prevent fragmentation.

What Will You Do?

As an Administrative Coordinator, you will be responsible for outreach and referral conversion, helping to ensure that eligible individuals are successfully enrolled and supported. You’ll also play a key role in providing essential administrative support to the clinical staff-including nurses, social workers, paramedics, and other team members.

Responsibilities

  • Conduct outreach to referrals and support enrollment
  • Communication with Referral Sources
  • Coordinate and schedule onboarding visits
  • Collect healthcare information from PCPs and partner agencies
  • Collaborate with the interdisciplinary team to ensure a seamless Participant experience
  • Maintain accurate records in the EMR system.

Schedule

  • Full-time, 40 hours per week
  • Weekend Schedule

Education and Experience

  • Minimum of 2 years of experience as a Medical Assistant or LPN
  • Experience with Microsoft Office software preferred.
  • Customer service experience in a health care setting highly valued.
  • Proof of a valid Michigan driver’s license, auto insurance and reliable transportation.
