The Admission Consultant will:

• Support the admission process to facilitate a smooth conversion from referral to admission by assessing the appropriateness for IRF, providing clinical information to SRAlab and communicating effectively with all parties. • Represent SRAlab rehabilitation services to various audiences within referral sources, primarily physicians and hospital discharge planners/case managers. • Review medical charts and communicate pertinent medical information to the internal medical team and external referrer when relevant. • Determine medical appropriateness of patients and support the admission process by gathering and transferring medical information to the appropriate inpatient admitting team. • Educate patients and families on the benefits of SRAlab and to facilitate the referral to admission. • Gather and use patient health information to assist as requested with treatment, payment, and healthcare operations. • Collect and analyze data and prepare reports related to customer service and throughput. • Monitor and report customer service issues to appropriate SRAlab personnel. • Manage the administrative aspects of customer service of a referral source by ensuring that databases reflect the referral/admission activities and maintain a current listing of all referral relations. • Partner with Admission Liaisons based in the field to coordinate the plan and admission when needed. • Perform all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab.

Reporting Relationships

• Reports directly to Director, Inpatient Access

Knowledge, Skills & Abilities Required

• Bachelors Degree and Current Illinois license to practice as a registered nurse required. • A minimum of five years of clinical nursing experience required; rehabilitation experience and CRRN strongly preferred. • Demonstrated ability to communicate clearly and knowledgeably with patients, families and clinicians to discuss treatment options. • Exceptional communication skills, both verbal and written. Understands excellent customer service and has successful proven experience in applying customer service skills. • Technically competent in using all electronic communications including a lap top computer, pagers and cell phones. • Highly organized, detail oriented, while understanding the big picture, and takes ownership to successfully meeting strict deadlines.

Working Conditions

• Normal office environment with little or no exposure to dust or extreme temperature.
