Come join our amazing team!Summary:The Admission/Wound Nurse is responsible for coordinating and providing comprehensive nursing care for newly admitted residents while overseeing wound assessment, treatment, prevention, and ongoing management. This position works closely with residents, families, providers, nursing staff, therapy, dietary, and other interdisciplinary team members to promote positive clinical outcomes and regulatory compliance.Key Responsibilities:Complete comprehensive nursing assessments and admission evaluations for new residents.Review hospital records, diagnoses, medications, orders, treatments, labs, and other clinical information prior to and upon admission.Coordinate the admission process and ensure accurate, timely completion of required clinical documentation.Develop and initiate individualized care plans based on resident needs, diagnoses, risks, and preferences.Complete comprehensive skin and wound assessments upon admission and throughout the resident's stay.Assess, measure, stage/classify, photograph as appropriate, and document wounds according to facility policy and current clinical standards.Develop and implement individualized wound treatment plans and monitor response to treatment.Perform wound treatments and ensure treatments are completed accurately and according to provider orders.Identify changes in wound condition, signs of infection, deterioration, or other complications and promptly notify the appropriate provider.Make recommendations regarding wound care products, pressure-relieving devices, positioning, nutrition, and other interventions to support healing.Monitor residents at risk for pressure injuries and implement appropriate prevention strategies.Collaborate with providers, dietitians, therapy, nursing staff, and other disciplines regarding wound management and nutritional or functional needs.Provide education and guidance to nursing staff regarding wound prevention, treatment techniques, documentation, and skin integrity.Conduct regular wound rounds and follow up on residents with active wounds or identified skin concerns.Monitor wound progress and update treatment plans and care plans as clinically indicated.Participate in interdisciplinary care conferences and communicate changes in resident condition or treatment needs.Ensure accurate documentation of assessments, treatments, measurements, progress, and interventions in the medical record.Assist with coordination of physician/provider appointments, orders, consultations, and follow-up related to admissions and wound care.Support medication reconciliation and identification of clinical concerns during the admission process.Review and communicate pertinent admission information with the Director of Nursing and nursing team.Assist with discharge planning and provide appropriate wound-care instructions and education when residents discharge.Monitor compliance with facility policies, state and federal regulations, and accepted standards of nursing practice.Participate in quality improvement activities related to wounds, pressure injuries, admissions, rehospitalizations, and clinical outcomes.Maintain confidentiality and protect resident rights while providing compassionate, person-centered care.Serve as a clinical resource and mentor for nursing staff regarding admissions, skin integrity, and wound management.Overall Purpose:To ensure that residents are safely and comprehensively assessed at admission and that wounds, skin integrity risks, and related clinical needs are identified early, appropriately treated, accurately documented, and continuously monitored throughout the resident's stay.Must have:Current Washington LPN or RN license without any enforcement actionCaldera Cares for you!Competitive pay scaleFull benefits package - Medical, Dental, and Vision401k Life and Disability InsurancePaid Time offGrow with Caldera Tuition Reimbursement Clinical ScholarshipsContinued EducationLeadership TrainingScholarship ProgramsPay Transparency Statement Compensation for roles at Spokane Falls Care varies depending on a wide array of factors including but not limited to the location, role, skill set and level of experience.
