Job Description:

Schedule: Full Time

Shift: Monday-Friday 1130a -8pm

Location: 480 W Central Parkway. Altamonte Springs, FL 32714

Assists in overseeing the admissions and referrals process to ensure efficient and timely patient intake. Communicates policies and procedures to staff to ensure compliance with federal, state, and local guidelines, as well as Medicare, Medicaid, and insurance reimbursement requirements. Interprets eligibility requirements for unique or questionable cases and provides guidance to staff. Coordinates with Clinical Services and Admissions Department leadership to determine and manage staffing levels. Oversees the handling of patient referral calls to ensure they are appropriately answered and processed. Communicates with healthcare providers, including doctors, hospitals, assisted living facilities, and skilled nursing facilities, regarding patient referrals. Monitors and tracks changes for non-admitted, discharged, or patients who have revoked the Hospice Medicare Benefit. Reviews nursing documentation and charting to ensure accuracy, timeliness, and completeness. Manages assigned direct reports including coaching, disciplinary actions, development, education, competencies, payroll functions, and documentation. Fills in for Admission Nurses as needed and completes admission visits when staff is unavailable. Participates in staff meetings, department meetings, team meetings, in-services, and other related activities. Knowledge, Skills, and Abilities:

Knowledge of Electronic Medical Records [Required]

Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software. [Required]

Knowledge of basic supervisory practices and procedures. [Required]

Ability to conduct interviews of prospective employees. [Required]

Ability to follow standard precaution guidelines for infection control, including when providing patient care. [Required]

Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]

Ability to complete Hospice of the Comforter Orientation and Training, as necessary. [Required]

Education:

Associate's of Nursing [Required]

Bachelor's of Nursing [Preferred]

Field of Study:

Graduate of an approved school of professional nursing.

in Nursing (BSN.

Work Experience:

1+ supervisory experience [Preferred]

3+ nursing experience including some hospice and/or home health care nursing. [Required]
