Admissions Coordinator
Rate: $20- $23/hour (depending on experience)
Shift: Full-Time
Schedule: Monday-Friday, 8 am - 4 pm
Sunset Nursing & Rehabilitation Center is looking to add an Admissions Coordinator to its team. In this role, you’ll help guide residents and families through the admissions process while building strong relationships with referral sources and ensuring a smooth transition into the facility. RESPONSIBILITIES:
- Coordinate and process admissions inquiries, referrals, applications, and requests for information
- Gather and review pre-admission information to determine level of care needs and admission eligibility
- Prepare and complete admission paperwork with residents and families and distribute documentation to appropriate departments
- Serve as a liaison between hospitals, discharge planners, referral sources, residents, and families
- Welcome and orient new admissions to the facility and provide tours to prospective residents and families
- Support facility census goals through marketing and relationship-building efforts
- Maintain admission records, logs, and documentation in accordance with facility policies and regulations
- Maintain HIPAA, infection control, safety, and corporate compliance standards
- Perform additional duties as assigned by supervisor
REQUIREMENTS:
- High School Diploma or equivalent required
- Associate degree preferred
- Minimum of 1 year experience working with residents and families in a long-term care setting required
- Strong communication, customer service, and organizational skills
- Ability to multitask and work collaboratively with interdisciplinary teams
- Compassionate and professional approach when working with residents and families
- Strong problem-solving and follow-up abilities
- Ability to maintain confidentiality and professionalism in a fast-paced environment
