We're looking for an outgoing, results-driven Admissions Coordinator/Care Navigator with experience in skilled nursing, long-term care admissions, or healthcare sales/marketing to join our team.

What You'll Do

As the face of our admissions process, you will play a critical role in both business development and resident experience:

Admissions & Intake Excellence

  • Lead the pre-admission process, gathering and analyzing clinical and financial data
  • Conduct interviews with patients and families to ensure accurate and compassionate intake
  • Complete admission documentation with precision and timeliness
  • Verify eligibility and enter data into EMR systems accurately

Relationship Building & Sales Focus

  • Build and maintain strong relationships with discharge planners, hospitals, and referral sources
  • Serve as a trusted liaison between referring facilities and our community
  • Identify and convert qualified referrals into successful admissions
  • Represent the organization with professionalism and enthusiasm in all interactions

Patient Served Growth & Strategy

  • Meet or exceed census goals through proactive marketing and outreach
  • Analyze admission/discharge trends and case mix to support marketing initiatives
  • Maintain a consistent pipeline through effective use of referral and intake systems

Operational Excellence

  • Manage admissions, transfers, and discharges in accordance with policies
  • Communicate census updates across departments
  • Maintain strict confidentiality and accurate resident records
  • Collaborate closely with the interdisciplinary team (IDT) to ensure seamless transitions

What You Bring

  • Associate degree in a business-related field or minimum 2 years of experience in long-term care admissions

  • Proven experience in:

  • Skilled nursing or post-acute admissions

  • Healthcare sales, marketing, or referral development

  • Working with discharge planners and hospital systems

  • Strong understanding of clinical eligibility and level-of-care assessments

  • Exceptional communication, organization, and relationship-building skills

  • Ability to think critically, move quickly, and prioritize effectively in a fast-paced environment
