    SUMMARY

    Coordinate the admission/transfer/discharge of patients in accordance with

    federal, state and local regulations, established procedural guidelines and as

    directed by your supervisor.

     

    LICENSURE/CERTIFICATION/EDUCATION REQUIREMENTS

    Minimum of a high school diploma, or its equivalent.

     

    OTHER TRAINING, SKILLS AND EXPERIENCE REQUIREMENTS

    Customer service oriented. Should possess good marketing and communication skills. On-the-job training provided in admission guidelines. Computer skills preferred

                                                                                                                                       

    ESSENTIAL SKILL/KNOWLEDGE FUNCTIONS

    • Participate in the pre-admission and admission process as well as arranging for

    transfer of patients to other centers should the need arise.

    • Obtain pertinent information from patients and/or family members, referral sources, and appropriate social agencies and verify financial information prior to admission and completes admission paperwork with family and patient.
    • Verify that the patient room, etc. is ready prior to admission.
    • Sustain contact and provide support to patient/families to include help in dealing with the family’s feelings and financial responsibilities.
    • Daily visits to new admissions to provide a smooth admission transition.
    • Orient the patient to the center environment.
    • Send follow-up thank you notes to families within one week of admission.
    • Meet frequently with other social workers and discharge planners within the community.
    • Maintain up-to-date information about agencies to which referrals may be made.
    • Maintain active waiting list file.
    • Communicate admission information to other departments.
    • Complete understanding of admission packet and ability to explain to family/patient.
    • Evaluate patient at hospital for appropriate placement as needed.
    • Knowledge of long-term care admission requirements.
    • Knowledge of advanced directives.
    • Knowledge and understanding of Medicare, Medicaid, Private Pay and Third Party reimbursement.

     

    ESSENTIAL ADMINISTRATIVE FUNCTIONS

    ·         Participate in Nursing Center surveys (Licensure / JCAHO) and any subsequently required reports.

    ·         Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.

    ·         Attend and participate in mandatory inservices.

    ·         Honor patient’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.

    ·         Comply with Corporate Compliance Program.

    ·         Report job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.

    ·         Follow established safety regulations, to include fire protection/prevention, smoking regulations, infection control, etc.

    ·         Follow established safety procedures when performing job tasks and/or working with equipment.

    ·         Perform other related duties as necessary and as directed by supervisor.

    ·         Comply with all Privacy & Security programs.

    PRIMARY ACTIVITY REQUIREMENTS

    Primary Physical Requirements:

    • Lift up to 10 lbs.: Rarely required when lifting patient medical files weighing approximately 5 to 10 pounds.
    • Lift 11 to 25 lbs.: Rarely may occur during course of work when stocking medical supplies or stocking medical cart.
    • Lift 26 to 40 lbs.: Not required.
    • Lift over 40 lbs.: Frequently lifting 40 pounds when assisting in patient transfer. (Occasionally may utilize the Hoyer lift when dealing with large or awkward-sized patients).
    • Carry up to 10 lbs.: Occasionally may be required to carry patient charts from one location in center to another (carry up to 100 feet, dependent on center size). When transferring several charts from one location to another, a cart can be utilized
    • Carry 11 to 25 lbs.: Not required
    • Carry 26 to 40 lbs.: Not required.
    • Carry over 40 lbs.: Not required.
    • Reach above shoulder height: 4 feet
    • Reach at shoulder height: Occasionally may occur when documenting information and compiling paperwork.
    • Reach below shoulder height: Occasionally may occur while retrieving patient charts from lower shelves.
    • Push/Pull: 20 feet.
    • Hand Manipulation:
    • Grasping: Frequently grasping pen/pencil when patient charts.
    • Handling: Constantly handling pen/pencil when documenting information into medical charts.
    • Torqueing: Not required.
    • Fingering: If center implements the use of a computer, may occasionally be required to use computer. Occasionally required to use a telephone.
    • Controls & Equipment: Computer, typewriter, telephone

     

    Other Physical Considerations:

    • Twisting: Not required.
    • Bending: Occasionally required when retrieving charts from lower shelves.
    • Crawling: Not required.       
    • Squatting: Occasionally required.
    • Kneeling: Not required.       
    • Crouching: Not required.   
    • Balancing: Not required.

    Cognitive and Sensory Requirements:

    • Sight: Constantly required when monitoring the outgoing and incoming visitors and patients and staff.
    • Tasting & Smelling: Not required..

     

    SUMMARY OF OCCUPATIONAL EXPOSURES

    Tasks assigned to position may involve potential and/or direct exposure to blood, body fluids, infectious disease, air contaminants and hazardous chemicals. May be subject to hostile and emotionally upset patients, family members, associates and visitors.

     

    OTHER CONSIDERATIONS AND REQUIREMENTS

    Should be able to communicate effectively, both verbally and in writing. The associate is required to lift a maximum of 5 to 10 pounds only. This position requires a considerable amount of walking. Associate must be able to drive a vehicle. Carrying is not frequently required as dollies are available. Associate should be able to sit and stand as tolerated.
